Ukraine: Helping Their Community Through Labor Therapy

The Ukrainian website Zapitya [ru], which has been promoting harm reduction programs in Ukraine has recently started a special blogging section [ru] where substitution therapy patients from various regions of Ukraine can share the personal stories and discuss issues of their concern.

One of the activists from the Substitution Treatment Advocates of Ukraine , Sergey Tetiorkin, has shared the story of how the substitution therapy patients from the city of  Ivano-Frankivsk (which is in Western Ukraine) started an initiative helping each other to overcome life challenges and even helping a community in which they live. At the beginning of the article, Sergey explained how this initiative emerged. He wrote

Для того, чтобы начать нашу историю, необходимо вернуться немного назад, в прошлое. В то время, когда только зарождалась программа заместительной терапии в нашем городе. А именно, в 2003-2004 гг., когда программа работала, в качестве «пилотного» проекта. Было нас 10-15 человек всего […] Но пути в программу, у каждого были свои. У кого проблемы милицией, у кого – со здоровьем (незаживающие абсцессы и трофические язвы на ногах). Кто-то выбился из сил. Но в основном всех объединяло одно – усталость! Усталость от тюрем, болезней и бега по замкнутому кругу. А тут такая возможность избавиться от этого «груза»! 

[…]Лечение было платным! 

И чтобы нам помочь, наш «папа», главный врач наркодиспансера, ввёл практику трудотерапии. Те, кто принимали участие в «лечении трудом», оплачивали 50% стоимости лечения. Для многих, кто сейчас твёрдо стоит на ногах, тогда это послужило отличным стартом к «новой» жизни. Люди вдруг вспомнили о том, что когда-то что-то оканчивали, где-то учились и работали. Вспомнили о том, что умеют кое-что делать своими руками! И надо сказать, что очень неплохие вещи получались. Вот собственно именно с этого и зародился наш так называемый «банк времени». 

To make my story clear for everyone, I should return a few years to the past, to the time when the substitution therapy program has just started in our city. It was 2003-2004 when our program began as a pilot project. It had only about 10-15 participants […]. Everybody had his own reasons for joining the program. Some people had issues with police; some had problems with health (abscesses and ulcers). …But there was something in common for all of us. We were tired of prisons and diseases which repeated again and again.  And we got the opportunity to get rid of all this!… 

[…] We had to pay for the treatment. 

To help us, our “Father”, the Chief Doctor of the clinic, launched a practice of a labor therapy. Those who participated had to pay only 50% of the treatment fee. For many of us it was a great start of the ‘new life’. People started recalling their old skills and knowledge and remembered that they can do something with their own hands….Actually this was a beginning of our so-call ‘time bank’.

Sergey explained exactly the exactly the patients helped each other: 

Вначале это была просто помощь один другому. Одна участница нашей группы пришла с проблемой. Опекунский совет поставил ей условие, что если она не изменит условия проживания, то остро встанет вопрос о лишении её «родительских прав». И действительно, после «деятельности» её супруга, который в то время отбывал свой очередной срок, их квартира превратилась в притон, где ребёнку не было места, и нуждалась в серьёзном ремонте. Так вот эта девушка обратилась ко всей группе с просьбой срочно помочь ей отремонтировать её квартиру. Как не помочь?! Засучив рукава, мы взялись за грунтовку, шпаклёвку, кисти и краски. Каждый приложил свою руку. И через неделю, квартира сияла и блестела! 

Потом ситуация повторилась. Но в этом случае супруг умер, … Шли месяцы. Программа продолжалась, становилась лучше, совершеннее. Лишнее отпадало, нужное приобреталось. Менялись и мы, первые участники программы заместительной терапии, мы становились сплочённее, дружнее! Разговоров о «ширке» становилось всё меньше, разговоров об обычной жизни, её проблемах и путях их решений, всё больше. […] 

At the beginning, it was just an assistance we provided one to another. For example, one participant shared her problem. The Guardian committee placed a condition that if she had not changed her lifestyle her baby would have been taken away.  Actually her husband who was in prison at that time had turned her apartment into a den where there was no place for a child and which required an urgent renovation.  So, this girl asked our group to help her with the renovation of her apartment. How could we say ‘no’? We took brushes, paints….in a week her apartment was like new.   

Later on, the story was repeated. We helped to renovate the apartment to another girl whose husband has died. A few months has passed. The program has become larger, and we, the first participants of the program had become real friends. … We talked less about drugs and more about ordinary life with its issues and ways of solving them. […] 

In 2004, Sergey’s clinic had problems with the provision of the medicationBuprenorphine which had some catastrophic consequences for the group. One of the patients was imprisoned, another had died.  The situation stabilized in 2008 when the harm reduction programs received support from the Ukrainian government. Sergey recalls: 

В тот год открылись программы по всей Украине, людей стало много, программа перевелась на государственный уровень, и препарат стал бесплатным. Как-то сама собой отпала необходимость в трудотерапии. 

Однако традиции и добрые отношения среди участников программы остались. Главный врач нашёл нам в новом здании другое, лучшее помещение. Огромный зал с евроремонтом, мебелью, туалетом и душем! А ещё там был аквариум и старый компьютер! 

Идея «Банка времени» возникла после просмотра телепередачи. Таким образом, мы решили поменять нашу трудотерапию на идею «банка времени». С приходом в программу новых людей, пришли и новые идеи. Одним из новых участников программы был Александр, ныне руководитель общественной организации «Запад-Шанс». Соответственно подбиралась команда единомышленников. Произошло слияние «стариков» (участников программы первого набора), с их традициями и отношениями и новичков (ребят только пришедших в программу), с идеями и энергией нового. 

In 2008 the programs opened all over Ukraine and the medication became free. There was no need for labor therapy any more. But the traditions and warm relations between patients remained in place. Our Chief Doctor provided us with new nice room. The big nicely renovate one, furnished, with a toilet and a shower!  There was also an aquarium and a computer in it! 

The idea of the ‘time bank’ we got from a TV show. So we decided to exchange our labor therapy with the ‘Time Bank’. With new people coming to the program new ideas appeared as well.  Alexandr, who now is a head of non-government organization ‘West-Chance’ was one of the new guys. Actually we got a good team of like-minded people. The ‘old’ guys (who were in the program from the very beginning) with their traditions and relationships matched well with ‘new’ guys who just joined the program with their fresh ideas and energy.   

 

Sergey described how the ‘time bank’ idea was elaborated and developed: 

Дело закрутилось! Нашли статью про группу молодежи из Скандинавии, которые имея совершенно разные интересы и профессии, объединились и начали помогать друг другу по принципу накопления и фиксации капитала. Только не в виде денег, а в виде трудочасов. Так решили поступить и мы. У большинства из нас образовалась масса свободного времени, которое раньше уходило на поиски денег и наркотиков, и мы очень хотели действовать, быть нужными и полезными друг другу и себе. Не могу сказать, что сразу всё пошло гладко и без проблем. Думаю, что редко бывает так, что затевая что-то новое, и это получается без труда, потерь и проблем… 

So we started. We found a newspaper article about a group of Scandinavian young people with different interests who united to help each other through accumulating resources.  But their resources were not money, but time in the form of labor hours. We decided to do the same. Most of us apparently had much free time which previously was used on search of money and drugs; besides we wanted to be active and to be helpful for each other. I cannot say that everything went smoothly from the very beginning. But it is not even possible that when you start something new that everything goes smoothly without hard work, challenges and losses[…]. 

Sergey believes that the ‘time bank’ initiative changed the attitude of the neighborhood towards the substitution therapy patients: 

Если ранее мы этим занимались как бы для себя, в свободное время, то теперь нам пора открывать фирму по оказанию услуг разной направленности. К нам приходят всё новые и новые люди. Не только участники программ ЗПТ. 

Узнав о нас, к нам потянулись жильцы соседних с сайтом домов, которые раньше были не довольны таким соседством. Потянулись к нам и врачи, помощь в ремонте (шпаклёвка, клейка обоев, покраска и т.д.) нужна многим! Мы научились и делаем откосы после установки евроокон и железных дверей, моем окна, грузим мебель, можем что-то выкопать, вывезти строительный мусор, садим деревья и кусты. Даже можем ухаживать за больными и детьми. Супруга Александра, парикмахер. Стрижет ребят прямо на сайте. Двое ребят прекрасно рисуют, помогают с поздравлениями. Творят картины, портреты, открытки. Есть парень, соображающий в компьютерах. Он обучает работе на компе. 

Before, we were doing this only for ourselves in our free time, but now we feel we need to start a business providing different kinds of services. New people join us. Not only patients of the substitution therapy. 

When people living in our neighborhood learned about us, they started using our services although earlier they were very unhappy having such neighbors like us.  Doctors also used our services, everybody who needed help in renovation (painting, hanging wallpaper, etc.)! We learned how to repair after the new windows or doors were installed, we clean windows, move furniture, dig, take away construction waste, plant trees and bushes. We also can take care of sick kids. The wife of Alexandr is a hair dresser. She cuts hair right on the site. Two of our guys can draw and help with greetings preparing special pictures, portraits and greeting cards.   There is a guy who is not bad in computers and he teaches others how to use it. 

Sergey is grateful to the medical personnel of the clinic for the support of their group: 

Не могу не рассказать о том, что наши медики очень нам помогают – административным ресурсом, консультируют тех из программников, у кого проблемы с алкоголем. К сожалению, сейчас проблема со злоупотреблением алкоголем стоит у нас довольно остро. Так вот, для таких пациентов, наши врачи организовывают курс лечения в стационаре, совершенно бесплатно… 

I also need to say that the medical personnel helped us very much with administrative issues and consultation of those who have alcohol problems. Unfortunately the issue of alcohol is very serious. So, the patients of the program can go though the alcohol addiction treatment free of charge.     

 

At the end Sergey share the plans for the future: 

А последняя наша задумка – это создание библиотеки и видеотеки. В ней мы хотим работать как положено, с формулярами, бланками и учётом. Благо материалов накопили (книг, дисков) достаточно много. 

Вся наша работа была записана в журналах и тетрадях, так вот, сейчас наш «компьютерный гений» работает над созданием специальной программы, при помощи которой удалось бы перенести все наши записи о том, кто и что делал для Банка времени в компьютер 

Our latest idea is the establishing a library of books and movies. We would like to organize it properly, with forms and a registration. Good that we have enough of books and CDs for it. 

Until now, all of our work was registered in books and notebooks. But now our “computer genius”  is developing a special software so we would be able to transfer our records on the ‘time bank’ into a computer.
